Action Required: Reporting

Refer to Admin. Procedure 5-02: Reporting Workplace Accidents.

Complete the School Administrator/Supervisor Accident Investigation Report and submit one copy to a Return To Work Coordinator and one copy to the Health and Safety Department within 24 hours.

The Health and Safety Department will contact the Ministry of Labour and the certified member of the Multi-Workplace Joint Health and Safety Committee representing the injured worker.

An on-site investigation will begin promptly. The Administrator/Designate, the health and safety representative (if available), the Health and Safety Officer (and/or Assistant Health and Safety Officer) and Certified Member representing the injured worker will examine the details of the incident and what possible actions can be taken to prevent any further accidents.
