ACTION REQUIRED BY Administrator / DESIGNATE

  1. Notify Emergency Response Team.

  2. Notify Superintendent of details of incident including:
    • what happened,
    • current status,
    • next steps, and
    • communication plan.
  3. If there is a critical injury, please refer to the instructions found under the critical injury section of this app.
  4. Access list of names, addresses and phone numbers of students on field trip if emergency contacts of students need to be notified.

IF NECESSARY

  • Notify the parent/guardian of the medical emergency. State the name of the hospital they have been taken to for medical care,
  • Arrange for transportation of students and staff from the scene to an alternate location such as a school to receive parents/guardians,
  • Arrange for staff to open school and receive parents/guardians, and
  • Communicate with OSBIE if an injury has taken place.

Debrief with staff, Superintendent and Emergency Response Team.
