Action required

Concussion is suspected without the presence of danger signs

  1. Contact parent/guardian regarding the injury and the need to pick up their child.
  2. Provide the parent/guardian with:
    • Concussion Medical Examination Documentation Request Form,
    • Letter of Accommodation for Suspected/Diagnosed Concussions,
    • Final Medical Documentation Form,
    • Copy Of completed Suspected Concussion Identification Tool, and
    • Inform parent/guardian that student cannot return to school for a minimum of 24 hours.
  3. Record, report and file accident as per AP 1-25: Injury/Accident Procedures.
  4. Submit an OSBIE Incident Report Form.
  5. Upon receiving Concussion Medical Examination Documentation Request Form and Letter of Accommodation for Suspected/Diagnosed Concussions from parent/guardian, post in the student’s OSR and have the Secretary enter the Suspected/Diagnosed Concussion in Trillium.

If a concussion is diagnosed by a medical practitioner:

  1. Initiate the process for a Return to Learn/Return to Physical Activity Plan and appoint a Collaborative Team. Notify all staff that work with the student that the student is on a Return to Learn/Return to Physical Activity Plan.
  2. Notify all school staff that work with the student that the student is on a Return to Learn/Return to Physical Activity Plan.
  3. Inform parent/guardian that when their child is no longer concussed, they must submit the Final Medical Examination Documentation Form to the Administrator/Designate in order for their child to return to daily activities with continued monitoring at school and by the parent/guardian at home.
  4. Upon receiving Final Medical Documentation Examination Form from parent/guardian, post in the student’s OSR and have the Secretary enter the Suspected/Diagnosed Concussion in Trillium.
  5. Ensure student is monitored while at school.

If a concussion is not diagnosed by a medical practitioner:

  1. Inform parent/guardian they must submit the Concussion Medical Examination Documentation Request Form to the Administrator/Designate in order for their child to return to daily activities with continued monitoring at school and by the parent/guardian at home.
  2. Ensure student is monitored while at school.
